Replacing Certificate of Citizenship or Certificate of Naturalization
Basic Requirements:
An application for a replacement Certificate of Citizenship or Certificate of Naturalization should be made under the following circumstances:
- If the Certificate of Naturalization or Certificate of Citizenship has been lost, mutilated, or destroyed; or
- If the U.S. Citizen changed his/her name by marriage or by court order and the name on the Certificate of Naturalization or Certificate of Citizenship has the prior name, or
- If the Certificate of Naturalization or Certificate of Citizenship has incorrect information.
